Is there an explanation of the db_pager functionality or are there examples?

No, not really. But look into /sales/inquiry/customer_inquiry to see an example at the end of the files. There are also examples of call back functions.
Also look into the internal files:
/includes/db_pager.inc
/includes/ui/db_pager_view.inc
