In my system, I can create a database only with phpMyAdmin and then give that to FA.

I can't reproduce the problem - company is created as required providing user/password db account has granted access to create new databases.
If you can't create database from php level you can still create database manually and then use it during company creation.

I installed 2.3 RC3. When I tried to add a company to the existing database I got error message "Unable to create database"
So I had to create a new database for each new company.

Today I installed 2.2.11 and now I can add new company to the existing database.
